Homes in the claimant's own name → verified; centre whose printed Provider Name is an organisation → ambiguous_match; another person's home → discrepancy; conditional / provisional / pending-revocation / fatalities / injuries / uncorrected findings are flags. Canaries 258416 (Adi Smith, FCCH) and 155813 (Bais Yaakov Early Learning Center @ Smith Avenue, ambiguous). Teachers are MSDE certification (not covered).
